Liability Waiver & Release

Please read the following waiver carefully and complete all fields before your session. A signed waiver is required to participate.

In consideration of being permitted to participate in surf lessons and related activities offered by Strait Surf Co (hereinafter "the Company"), I, the undersigned, hereby acknowledge, agree, and represent as follows:

1. Assumption of Risk

I understand and acknowledge that surfing and surf instruction involve inherent risks, dangers, and hazards including but not limited to: drowning, collision with other surfers or watercraft, impact with the ocean floor or reef, strong currents, rip tides, unpredictable wave conditions, sunburn, and physical injury. I voluntarily assume all such risks, both known and unknown.

2. Release of Liability

I, on behalf of myself, my heirs, assigns, and next of kin, hereby release, waive, discharge, and covenant not to sue Strait Surf Co, its owners, instructors, employees, agents, and volunteers from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, injury, or death that may be sustained by me while participating in surf lessons or while on the premises.

3. Medical Fitness

I certify that I am physically fit and have no medical conditions that would prevent my safe participation in surf lessons. I agree to inform my instructor of any medical conditions, injuries, or physical limitations prior to the start of my lesson.

4. Indemnification

I agree to indemnify and hold harmless Strait Surf Co and its representatives from any loss, liability, damage, or costs, including court costs and attorney fees, that they may incur due to my participation in surf lessons, whether caused by negligence or otherwise.

5. Rules & Instructions

I agree to follow all safety rules and instructions provided by Strait Surf Co instructors at all times. I understand that failure to follow instructions may result in removal from the lesson without refund.

6. Photography & Media

I grant Strait Surf Co permission to photograph or video record me during lessons and to use such images or footage for promotional and marketing purposes, including on social media and the company website, unless I indicate otherwise below.
